Hospice Uses a Team Approach

Many people with different skills work as a team to provide Hospice care. Team members include:

Physicians: Are an important part of the team and the Hospice Medical Director is always available for consults.

Registered Nurses: Make regular visits to manage care. An on-call nurse is available 24 hours a day for phone calls or visits.

Licensed Practical Nurses: Help with personal-care needs.

Social Workers: Help patients and families with death and dying issues, both practical and emotional.

Grief Counselors: Help families cope with grief and loss issues and continue to support families thirteen months after the death.

Chaplains: Can offer spiritual support for patients and families and help deal with end-of-life questions.

Trained Volunteers: Can be a big help by running errands, sitting with a patient when a caregiver is unable or by just listening.

Other Experts: Dieticians, physical and occupational therapists as well as speech therapists are available as needed and can help to improve quality of life for some patients.
